Please be sure to fill out the following forms and bring them to your first session. This will help ease you into therapy, and allow as much time as possible to be spent focusing on you.
We accept cash, check and all major credit cards as forms of payment. Payment is collected during the session.
Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.
We recommend asking these questions to your insurance provider to help determine your benefits:
- Do I have out-of-network benefits to cover outpatient psychotherapy services?
- Do I have a deductible? If so, what is it and have I met it yet?
- What is the percentage of reimbursement for code 90834 (45-minute session)?
- What is the percentage of reimbursment for code 90791?
- Do I need written approval from my primary care physician in order for services to be covered?
If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged for the full rate of the session.

The Center for Effective Psychological Services PLLC is hosted inside the NAYEC building on the first floor. You will be required to check-in at the front desk. You can sign with your initials.
Directions & Parking Information
Metro: Our office is conveniently located near the Metro, 2 blocks from the McPherson Square Station on the Blue/Yellow Line and 4 blocks from either Metro Center or Farragut North on the Red Line.
Parking: There is limited street parking on 13th, 14th and L Streets, NW (please be mindful of the restrictions noted on the street signs).
